Prescription Refills


Prescriptions are refilled through one of three methods.

1. Is this prescription one that you pick-up at Broadway Veterinary Hospital? If yes, you may continue here to request your refill.

2. Is this prescription one that is filled through a human pharmacy, Ballard Plaza Pharmacy, QFC Pharmacy, Walgreens Pharmacy, etc? If yes, please call your pharmacy directly to request your refill. The pharmacy will fax your request to Broadway Veterinary Hospital and once the doctor has reviewed your request we will fax back to your pharmacy.

3. Is this prescription one that you wish to fill online? If yes, please visit VetSource for home delivery.  

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's prescription by submitting the following form. Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor.

We will notify you by phone when your pet's prescription is approved and ready to be picked up.
